The Spanish Peaks Outreach Clinic is an off campus, outpatient, safety net clinic that provides health and wellness programs to many of our under-served population. We offer many opportunities for the community to receive free or low cost services that encourages healthy living through creating healthy life styles. Programs that compliment and support health and wellness, from children to the elderly, are offered that can help them navigate their way to a healthy life style!
The Clinic is the recipient of several health grants from the Colorado Department of Health and Environment (CDPHE) including the Title X Family Planning Program and the Women’s Wellness Connection. Through these grants we are able to offer women and men birth control, family planning counseling, STD and HIV testing, wellness exams, mammograms, vasectomies and Essures. We can also provide PSA’s for men at a minimal cost. For eligible patients interested in a colonoscopy we are able to navigate the service at no cost. We emphasize the importance of yearly checkups as a tool to prevent cancer.
Cardiac assessments including lipid panel, glucose testing, pulmonary function testing and BMI are available. In addition we have a nutritionist on site that works with patients that have high BMI’s and or other chronic illnesses to improve their health and quality of life. Mental Health counseling for families is also available at no cost.
In an effort to create a healthier community the Outreach Clinic also has a Colorado Health Foundation Grant that provides well child screenings and services to all Huerfano County youth. This wonderful grant provides comprehensive well child checks, follow up care, mental health screening, nutrition counseling, vaccinations, lipid panel and lead screening. The Outreach clinic is now participating in the LiveWell CO initiative which will assist in the development of health lifestyles.
Complimenting our Well Child services are two grants that address early childhood education. One from Catholic Charities (TGYS and DBH) and one from Colorado Children’s Trust Fund that works with parents of young children offering Early Childhood parenting classes (Nurturing Parenting). Participants receive a weekly stipend, lunch or dinner, childcare while taking the class, and Red Cross certification. Parents are taught alternative disciplines, potty training, nutrition, and about healthy growth and development of their children. Bright Beginnings is another component of our early childhood program that concentrates on the importance of early literacy. Both programs provide parents opportunities to raise happy and healthy children.
The Pueblo Community Health Center sends a Midwife and nurse once a week to meet expecting parents’ prenatal needs. There is a Nurse Family Partnership program that provides an RN during pregnancy and continues until the baby is 24 months old. (Where else can you get a private nurse for almost three years for free??) Dr. Dennis Driscoll from House Family Dental Pueblo comes once a month to provide dental care for children ages 1-18 years old. Dr. Driscoll accepts Medicaid and CHP+.
We have two new programs that support wellness. The first is a Community Wellness Case Manager who will work with patients discharged from the hospital and or referred by a provider for chronic illness management. The second is the LiveWell grant that will support Healthy Eating and Active Living activities throughout Huerfano County.
Voice Care and Med Time are available for residents in our region that require additional security for falls and or those in need of medical attention. Med Time provides the opportunity for timed medication.
For clients with no insurance or resources to pay for medications we offer Prescription assistance for free or reduced medications and presumptive eligibility for Medicaid and CHP+. We are also a referral agency for Quit Line and offer tobacco cessation classes through the American Lung Association. Healthy activities are available throughout the week including adult and children’s tap, Zumba Aerobics, and Karate and are provided in the “Ball Room” on the top floor of our facility.
